Upon the completion of this form you will receive a copy of NCWDI constitution. The original copy of this form is kept with other NCWDI's official documents; however, a copy of the signed form will also be provided to you upon request.

• The first six members are considered as the Executive Members of the executive committee.
• The President of the committee is elected from the executive members every two years.
• Upon departure of an Executive Member, other members may submit their request to join the executive committee.
• The terms and conditions of executive membership may be subject to change every two years. All members will be notified through NCWDI electronic bulletin.
